All personal expenses including hotel room service and meals; however, you have the option to pay for meals that tour guide arranges.
Yes, president museum is an optional tour that you may attend. (President Museum ($21.00 per adult, $15.00 per child) )

This tour is available on Dec 3-5. Unfortunately, it is not possible for me to arrange you a train back by Eurostar back to the airport. You would need to self-transfer yourself back to the airport.

The place to meet at the start of the tour would be either the Airport or the first night's hotel. As for Day 4, there will not be a stop at the Hoover Dam, however if you would like to stop by, you may request for a stop with the tour guide, however there is no guarantee that there will be a stop.
